To join the Maryland Titans for the 2008 season, please select the appropriate form below.
New Team Members

New athletes must also provide a copy of their birth certificate or passport as proof of age.
As part of the registration process, each athlete will also need to print and complete the following forms:
- Waiver, Release, and Statement of Physical Condition
- Athletes Contract
- Parents Code of Conduct
